Plum Creek Assisted Living is a family, locally and veteran-owned community that was built in 2009. Guided by our owner, Dan Ihrig, LNHA, we are deeply committed to ensuring that every resident receives the highest quality of care in a warm, supportive environment.
Our community offers far more than a place to live; it’s a place to thrive. Residents enjoy a variety of amenities and services designed to support their health, independence, and happiness, including a full-service beauty salon, restaurant-style dining, engaging social activities, and specialized exercise programs. We also provide personalized care services, skilled nursing support, and comprehensive rehabilitation through certified physical, occupational, and speech therapists.
Plum Creek is licensed by the Ohio Department of Health and certified by the Ohio Department of Aging to provide basic services. We utilize the Medicaid Assisted Living Waiver as a payor source for current residents who have exhausted their personal resources.

Our owner, Dan Ihrig, LNHA, brings over 40 years of experience in senior care and a deep commitment to serving others. His leadership reflects the same values of dedication, integrity, and service that guided his time in the military.
